A well-established, mission-driven Federally Qualified Health Center (FQHC) serving the Chicago area is seeking a
bilingual Family Nurse Practitioner (FNP) or Physician Assistant (PA)
to join its primary care team.
Polish language fluency is absolutely required.
This position serves a large Polish-speaking patient population, and candidates
must be fluent in both English and Polish . Unfortunately, candidates who do not speak Polish cannot be considered.

Position Highlights
Full-time Family Nurse Practitioner (FNP) or Physician Assistant (PA)
MUST be fluent in both Polish and English (non-negotiable requirement)

Clinic Hours
Monday: 6:30 AM 8:00 PM
Tuesday: 8:30 AM 5:00 PM
Wednesday: 6:30 AM 8:00 PM
Thursday: 7:30 AM 8:00 PM
Friday: 8:30 AM 5:00 PM
Saturday: 8:00 AM 2:00 PM
Qualifications
Graduate of an accredited Nurse Practitioner or Physician Assistant program
Board Certified with certification maintained in good standing
Active or eligible Illinois APRN or PA license
Illinois Controlled Substance License and DEA registration
Current provider-level CPR certification
Fluent in both Polish and English (required)
Compensation & Benefits
Competitive salary:
$97,000 $130,000 , based on experience
Full Practice Authority NPs start at a higher salary scale
$20,000 salary increase
after one year for eligible providers meeting productivity/template requirements
